1. Interested candidates fulfilling the eligibility criteria for Sl. No. 06 & 07 can apply to the Chief District Medical & Public Health Officer, At/Po- Paralakhemundi, Gajapati, 761200 on or before 30.10.2021 by 05.00 PM through Registered Post/Speed Post/Courrier only and the envelope containing the application should be superscribed with the name of the post applied for Application form for each individual post must be submitted in separate format along with attested photocopy of all the mark Sheet & Certificates.
2. For Post Sl. No. 01/02/03/04/05 the candidates are requested to Register their name from 10.00 AM to 11.30 AM to attend the Walk-In-Interview at the District Training Unit (DTU), O/o. CDM&PHO, Gajapati.
