- ପ୍ରାର୍ଥନୀ ମାଟ୍ରିକ ପାସ୍ ହୋଇଥିବା ଆବଶ୍ୟକ । ଯଦି ମାଟ୍ରିକ ପାସ୍ ପ୍ରାର୍ଥିନୀ ନ ମିଳନ୍ତି ତେବେ କୌଣସି ସ୍ଵିକୃତୀପ୍ରାପ୍ତ ଉଚ୍ଚ ବିଦ୍ୟାଳୟରୁ ଅନ୍ୟୁନ ଅଷ୍ଟମ ପାସ୍ (ତଫସିଲଭୁକ୍ତ ଜନଜାତି) କରିଥିବା ପ୍ରାର୍ଥନୀଙ୍କୁ ବିଚାରକୁ ନିଆଯିବ ।

- ଜାତିଗତ ପ୍ରମାଣ ପତ୍ରର ସହି ନକଲ । (ସମ୍ପୃକ୍ତ ତହସିଲଦାରଙ୍କ ଠାରୁ ପ୍ରାପ୍ତ ହୋଇଥିବା ଆବଶ୍ୟକ)
- ଶିକ୍ଷାଗତ ଯୋଗ୍ୟତା ପ୍ରମାଣ ପତ୍ର ଓ ମର୍କସିଟ୍ ଅବିକଳ ନକଲ ।
